Walk-in Convenience When You Need It

We understand the importance of timely medical care. That's why we welcome both new and established patients to visit us for walk-ins Monday through Thursday, 7:00 a.m. to 4:00 p.m. 

Lab Services

In addition to our walk-in clinic services, we continue to offer comprehensive laboratory services. Whether you require routine lab tests or specialized diagnostics, our facility is equipped to handle your needs efficiently. Lab services are available both by appointment and on a walk-in basis.

Meet our Team


Rebekah French, DNP, FNP-C

Dr. French earned her Bachelor of Science degree in Nursing from Northern Arizona University, a Master of Science in Nursing specializing as a Family Nurse Practitioner from Graceland University's School of Nursing in Independence, Missouri, and a Doctor of Nursing Practice from the University of New Mexico. Ms. French is from Aztec and has been practicing nursing at San Juan Regional Medical Center since 2009. In addition to caring for patients in her hometown, she also serves on the Aztec Municipal School District’s Board of Education.

Miki Moffitt, CNP

Miki Moffitt, CNP

Ms. Moffitt is a proud graduate of Farmington High School and San Juan College, where she earned her associate degree in nursing. She holds a bachelor’s degree in nursing from the University of Wyoming, and a master’s degree as a Family Nurse Practitioner from Grand Canyon University. Ms. Moffitt has a long history of serving patients in our community, with more than 28 years of experience as a nurse, most recently at our San Juan Health Partners Urgent Care clinic.

Tara Smith, CNP

Tara Smith, CNP

Ms. Smith is a graduate of Aztec High School. She received her Bachelor of Science in Nursing from New Mexico State University in Las Cruces in 2008. She came back to San Juan County and worked as a nurse in the ICU at San Juan Regional Medical Center from 2009-2014. While still working full time, Ms. Smith went back to school and recently received a Master of Science in Nursing: Family Nurse Practitioner from Graceland University in Independence, Missouri.

Jessica Ann Conklin, FNP-C, PMHS

Jessica Ann Conklin, FNP-C, PMHS

Jessica was born and raised in Farmington and attended the University of New Mexico. She graduated with honors in 2007 with a bachelor of science in nursing. She went on to complete her master's degree as a Family Nurse Practitioner from Graceland University in Independence, Missouri in 2016.
