Physical Therapy
Jacob Harris

Jacob Harris, PT, DPT

Physical Therapist

Dr. Jacob Harris was born and raised in Farmington, New Mexico. He earned his Bachelor of Science in Biology from Brigham Young University–Idaho and his Doctor of Physical Therapy degree from Rocky Mountain University of Health Professions. Jake is a certified LSVT BIG clinician, specializing in the treatment of individuals with Parkinson’s disease. He is also trained in concussion management through ImPACT, is a Graston Technique Specialist, is certified in functional dry needling, and is fluent in Spanish. Jake is committed to providing personalized, evidence-based care to help his patients return to the activities that matter most to them.

Deborah French

Deborah French, PT, ATC

Physical Therapist, Certified Athletic Trainer

Debbie brings over 35 years of experience in physical therapy and athletic training to the clinic. She has been a physical therapist and certified athletic trainer since 1990. She completed her undergraduate education at Southern Oregon College, where she participated in track and field for two years and basketball for four years, and then completed her physical therapy education at the University of California, San Francisco School of Medicine.

She worked in an outpatient sports medicine clinic for several years in California before returning to school to complete a Master’s degree in Sports Management at The Ohio State University. Over the course of her career, Debbie has worked in outpatient orthopedics, skilled nursing facilities, and home health care, as well as covering high school athletic events.

She has completed many continuing education courses over the years, including Graston therapy and strain-counterstrain manual therapy. She also has a strong interest in sports, having coached high school basketball and served as an athletic trainer for many years while living in Petaluma, California. She has been working for SJRMC since 2020.

Stephanie Salisbury

Stephanie Salisbury, PT, DPT

Physical Therapist

Stephanie earned her Doctorate in Physical Therapy from Wichita State University in 2016, after completing her Bachelor’s degree in Exercise Science. She brings a diverse clinical background, with experience treating patients across pediatric, geriatric, and neurological rehabilitation settings.

Stephanie is certified in Vestibular Rehabilitation through the American Institute of Balance and is also LSVT BIG certified, specializing in evidence-based treatment for individuals with Parkinson’s disease and other neurological conditions. She has a particular interest in working with patients experiencing dizziness and balance impairments, helping them regain confidence, improve stability, and return to their daily activities.

Stephanie is dedicated to providing individualized, patient-centered care focused on achieving meaningful, functional outcomes. Outside of the clinic, she enjoys spending time doing anything outdoors, paddle boarding, and having fun with her family.

Bria Rubie

Bria Rubie, PT, DPT

Pelvic Health Physical Therapist

Bria graduated with her Doctorate of Physical Therapy degree from Rocky Mountain University of Health Professions in 2022. She began practicing in Durango, Colorado, before starting at SJRMC in 2024. Bria enjoys treating a wide variety of conditions, including pelvic health, and is certified in Functional Dry Needling. She is originally from Salt Lake City, Utah, and loves the Four Corners region with its mountains and desert landscapes. During her time off, you can find her outside with her husband, son, and dog either hiking, biking, backpacking, or skiing.

Amy Link

Amy Link, PT, DPT

Pelvic Health Physical Therapist

Amy graduated with her doctorate in Physical Therapy from the University of New Mexico in 2018. She began her pelvic health training in PT school and began treating pelvic health conditions upon graduation. In 2021, she received her Pelvic Rehabilitation Practitioner Certification. She enjoys helping her patients recover from pelvic floor dysfunction while providing a safe and comfortable space for healing.

Amy grew up in Farmington and moved back after spending 15 years in Albuquerque. She loves to spend her time gardening, being outside, and spending time with her husband, son, and two dogs.

Jessica McConkie

Jessica McConkie, LPTA

Rehabilitation Outpatient Center Coordinator

Jessica McConkie is a Licensed Physical Therapist Assistant who earned her Associate of Applied Science degree from San Juan College in 2015. She has been with San Juan Regional Medical Center for 10 years, beginning her career at the San Juan Regional Rehabilitation Hospital before transitioning to the Rehabilitation Outpatient Center, where she now serves as Coordinator.

Jessica has a special interest in treating individuals with neurological conditions, including traumatic brain injury, cerebrovascular accident/stroke, and spinal cord injuries. She is a certified Fall Prevention Specialist and a certified LSVT BIG clinician, specializing in treatment for individuals with Parkinson’s disease and other neurological disorders.

Outside of work, Jessica enjoys spending time with family and friends, camping, and exploring the outdoors.

John Corley

John Corley, LPTA

Physical Therapist Assistant

John graduated with an associate’s degree as a Physical Therapy Assistant from San Juan College in 2015. He has worked primarily in the outpatient orthopedic/sports medicine setting, working with all levels of functional ability. He enjoys working with orthopedic patients and people looking to make a positive impact on their quality of life through physical therapy and movement.

John has completed certifications in Graston Technique, Mulligan Technique, Concussion Rehabilitation, and LSVT for Parkinson’s. John enjoys outdoor activities and spending time with his wife and kids and is a founding member of the ROC Book Club and ROC Board Game Club.

Grace Chavez

Grace Chavez, BS, LPTA

Physical Therapist Assistant

Grace was born and raised in Farmington, New Mexico. She earned her Bachelor of Science in Exercise Science from Fort Lewis College in Durango, Colorado, and her associate’s degree as a Physical Therapist Assistant through the San Juan College PTA program. She began her career at San Juan Regional Medical Center’s rehab outpatient clinic in 2022.

Grace is a certified LSVT BIG clinician and has a special interest in treating individuals with Parkinson’s disease. She is also trained in Graston Technique, blood flow restriction therapy, fall prevention, and concussion management. In addition, she provides aquatic therapy, using the pool to support rehabilitation, reduce pain, and improve mobility for a variety of conditions.

Grace is passionate about helping her patients improve their strength, mobility, and confidence so they can return to the activities they enjoy most. Outside of the clinic, she enjoys spending time with her husband and animals and exploring the Four Corners region.

Jessica Shorty

Jessica Shorty, LPTA

Physical Therapist Assistant

Jessica Shorty was born and raised in Shiprock, New Mexico. She is a licensed Physical Therapist Assistant who graduated from San Juan College in 2017 with an Associate of Applied Science in Physical Therapist Assistant. Licensed since August 2017, she has experience working with patients of all ages and is passionate about helping others improve their mobility, confidence, and quality of life. Jessica is certified in LSVT BIG and Fall Prevention, specializing in movement restoration and balance training.

Rachel VanHerwaarden

Rachel VanHerwaarden, LPTA

Physical Therapist Assistant

Rachel was born and raised in Farmington and has been a licensed Physical Therapist Assistant since 2017. She is a Certified Fall Prevention Specialist with advanced training in Graston Technique and LSVT BIG for Parkinson’s patients. Rachel is dedicated to providing high-quality, patient-centered care and is passionate about helping individuals improve their mobility, function, and overall quality of life.

She values being an active member of the community and supporting patients throughout their rehabilitation journey. Outside of the clinic, Rachel enjoys spending time with her husband and two children, traveling, camping, and baking.

Occupational Therapy
Sean Fusco

Sean Fusco, OTR

Occupational Therapist

Sean graduated from Chatham University in 1999 with a Master of Science in Occupational Therapy. Sean has more than 25 years of experience specializing in hand, wrist, and elbow trauma, post-surgical rehabilitation, and custom splint fabrication. Sean has been a caregiver at SJRMC since 2011.

Adina Velarde

Adina Velarde, COTA/L, CFPS

Certified Occupational Therapy Assistant

Adina was raised in Dulce, New Mexico, and graduated from Escalante High School in Tierra Amarilla, New Mexico. She received her Associate of Applied Science as an Occupational Therapy Assistant in 2021 from San Juan College. She worked in acute care until her transition to outpatient rehab in 2024 to specialize in hand therapy.

Adina is an active member of the Occupational Therapy Assistant Program Advisory Board with San Juan College, keeping in touch with program updates, fieldwork, and employment opportunities for upcoming graduates. In her free time, she enjoys playing video games with her son, baking, going to the gym, and reading.

Speech Language Pathology
Karen Brack

Karen Brack, MS, CCC-SLP

Speech-Language Pathologist

Karen is a born and raised Farmington local. She holds two bachelor’s degrees and a master’s degree from Eastern New Mexico University. She graduated from Eastern New Mexico University with her Master of Science in Communicative Disorders with an emphasis in Speech-Language Pathology in 2024. She currently holds a speech-language pathology license in New Mexico and a certificate of clinical competence through the American Speech-Language-Hearing Association.

Karen currently provides outpatient adult and pediatric evaluations and treatments. She continues to seek more educational certifications to provide the best care for her patients.

Karen’s special interests include evaluation and treatment of adults with aphasia, dysphagia, and traumatic brain injury. In her free time, Karen enjoys spending time outdoors with her family, friends, and dog, or staying in for game night.
