Diagnoses Served
Admission to the San Juan Regional Rehabilitation Hospital is not dependent on any specific diagnosis. Each patient's medical information, needs, ability to perform daily activities, and potential to improve are considered. However, one must meet admission criteria.

Discharge
Your rehabilitation team will estimate the length of your stay and provide training for you and your family to ensure a safe transition home. Training times will be coordinated with your caregivers.
Your case manager will assist with arrangements for care after discharge, such as home health services, outpatient therapy, and doctor appointments.
Discharge Equipment:
Before discharge, necessary equipment (e.g., wheelchair, walker, bath bench) will be recommended. Some items may not be covered by insurance; your therapists and case manager will discuss your options.
It is best to wait before purchasing equipment or remodeling your home until you have consulted with your therapists.
