Menopause and Hormone Therapy

Few stages of life are surrounded by as much confusion and conflicting information as menopause. Many women feel unsure about what’s normal, what’s treatable, and what actually requires medical evaluation. Dr. Suzanne Ciotti sees this firsthand in her practice at San Juan Health Partners Family and Specialty Medicine. She addresses the most common misunderstandings she sees in clinic.

Misconception #1: Hormone Testing Is Always Necessary in Perimenopause

“Hormone testing is extremely tricky,” she explained. “If you think about a woman’s cycle, hormones are going up and down normally. On any specific day, levels may be low — and that might be completely normal.”

Because of natural fluctuations, testing during perimenopause isn’t always helpful for guiding treatment.

Misconception #2: Bioidentical Hormones Are Completely Different

“Patients often come in asking for bioidentical hormones,” Dr. Ciotti said. “But many of those contain the same ingredients that are in regular prescriptions like estradiol and progesterone.”

The difference is often in dosing flexibility and delivery methods.

“We can sometimes prescribe them in smaller increments or different forms like creams, capsules, or vaginal treatments.”

Final Thoughts from Dr. Ciotti

Hormonal transitions can be difficult — but women don’t have to navigate them alone.

“I think it’s a good idea to have a provider who is knowledgeable about different treatments,” she said. “If you feel like you’re not getting the right answers, reach out. There are more providers becoming knowledgeable about hormone treatments at all ages.”
