View the 2023 Community Health Needs Assessment and Implementation Plan
A Community Needs Assessment survey is conducted by San Juan Regional Medical Center and San Juan Regional Rehabilitation Hospital to determine the health needs of San Juan County as perceived by the citizens of the county.
Our hospitals' Boards of Directors and management team conduct this survey to determine how best to direct resources to improve the health of all citizens in the county. Gaps in healthcare are identified and evaluated, and the hospital's focus will be on closing those gaps.
We are pleased to present the results of our comprehensive Community Health Needs Assessment conducted in 2023.
Through the insights gained from the Community Health Needs Assessment, we have prioritized three key areas:
- Access to Healthcare
- Diabetes
- Cancer
Over the next three years, we will work diligently to bridge the gaps in healthcare access by collaborating with stakeholders, advocating for policy changes, and implementing initiatives that ensure everyone in our community can receive the care they need when they need it.
Diabetes is a prevalent chronic condition that significantly impacts the lives of many individuals. Our goal is to empower individuals to make informed choices, promote healthy lifestyles, and enhance diabetes management through innovative approaches and community partnerships.
Cancer remains a significant health challenge, affecting countless lives within our community. In the coming years, we will prioritize initiatives that focus on cancer prevention, early detection, and comprehensive treatment options.
We are committed to working tirelessly to meet these community needs head-on. Together, with the support and involvement of our community members, we can make a lasting impact on the health and well-being of all individuals in our community.
