Making an advance directive—which is a written set of instructions that can be placed in your medical record—helps you be in control of treatment decisions and also helps your family, friends, and physicians follow your wishes if you are unable to act following a serious illness or injury. For more information about advance directives, talk with your nurse, physician, or a member of the Care Facilitation Team at 609-6260.
